Admission Procedure

A. Direct to Room Admission:

 

Step 1. Patient or relative makes arrangements with the Admitting Section. 

 

Step 2. The Admitting Officer gathers all information required for admission.

 

Step 3. The Admitting Officer provides the patient with leaflets containing the patient’s rights and responsibilities and informs the patient of these rights and responsibilities. Patient learns of his/her rights and responsibilities upon admission.
 

Step 4. The Admitting Officer secures the Consent for Admission/Management /Procedure.

 

Step 5. As the registration procedure is completed, the Admitting Officer provides the information about the availability of rooms, hospital policies, and procedures.

 

SPECIAL CONSIDERATION
 

  1. Patients with communicable diseases must be admitted to the Isolation/ Private Room.

  2. Requests for confidentiality or privacy may be accommodated.

 

Step 6. All patients admitted are informed that a wrist bond (pink for female and blue for male) must be worn around the wrist for the duration of their stay in the hospital. Information written on the wrist bond includes the full patient’s name, age, birth date, and case number.
 

Step 7. Admission kit will be given upon admission.

 

Step 8. The nurse accompanies the patient to the Emergency Department for proper assessment by Resident on Duty, who takes and records initial vital signs and proceeds with history taking. The Resident on Duty notifies the Attending Physician of admission.

 

Step 9. The nurse informs the Nurse on Duty in the ward about the admission. The information to be given includes the admitting impression or diagnosis, room of choice, and the name of the Attending Physician for initial endorsement.

 

Step 10. Orders for laboratories, Chest X-Ray, CT scan, and ECG are done before taking the patient to the room of choice.

 

Step 11. The nurse and orderly transport the patient on a wheelchair or stretcher to the preferred room.

 

Step 12. The Ward Nurse on Duty who receives the patient:
 

  1. Welcomes and accompanies the patient to the room of choice and makes him/her feel comfortable.

  2. Orients the patient and relative to the environment, hospital rules, and regulations.   

  3. Performs initial assessment and brief history taking.

  4. Informs the Attending Physician of admission as well as the room number.

  5. Informs other departments, such as the Operating Room, X-Ray, CT-scan, Ultrasound, and MRI of special procedures to be done, as and when needed.

  6. Carries out doctor’s orders, transcribes in KARDEX, and makes the necessary documentation. 

 

B. DIRECT ADMISSION TO DELIVERY ROOM/ OPERATING ROOM

 

Step 1. The patient/relative makes arrangements with the Admitting Section (Out-Patient Department Information).

Step 2. The Admitting Officer secures the Consent for admission/management/procedure from the patient.

​

Step 3. The nurse performs vital signs assessment and recording.

​

Step 4. The nurse informs the Operating Room department regarding the admission.

​

Step 5. If needed, the nurse accompanies the patient to the Emergency Room for proper clinical evaluation and history taking.

Step 6. The nurse and orderly accompany the patient to the Delivery Room/Operating Room.

​

Step 7. The Delivery Room/Labor Room/Operating Room nurse on duty who receives the patient:

  1. Welcomes and accompanies the patient to the Delivery Room/ Operating Room.

  2. Makes an initial nursing assessment and brief history taking.

  3. Orients the patient/ relative to the environment, hospital rules, and regulations.

  4. Informs the Attending Physician about the admission.

  5. Informs other departments, such as Operating Room, X-ray, Laboratory, Ultrasound, among others, for any special procedures to be done.

  6. Carries out doctor’s orders, transcribes in KARDEX, and makes the necessary documentation.
